CarlSmile is astronomer Carl Sagan, host of Cosmos: A Personal Voyage. The emote entered Twitch culture during the platform's April 2017 Science Week, which opened with a marathon of all 13 Cosmos episodes and continued with programming involving NASA, CosmoQuest, and Cosmos co-creator Ann Druyan.

The grayscale image shows Sagan looking upward with a calm smile. By June 2017 chat was already using CarlSmile in a mock-scientific Imaqtpie copypasta about a white shirt, gray screen, and "empirical data" identifying a QTpie stream. In SaltyBet, CarlSmile later became closely tied to `!dbz` and the old betting maxim "Always bet on DBZ," often used with deliberate, weary irony after Dragon Ball Z characters became a familiar betting trope.

Viewers type CarlSmile in Twitch chat for Carl Sagan, Cosmos, astronomy, and science references, or for a calm reaction to something predictably familiar. In SaltyBet culture, it is also the established punchline to the ironic "Always bet on DBZ" refrain.
